A patient assessment is always carried out:
•Upon initial admission (to ER and ward)

when is a quick priority assessment completed?

A

In hospital it is done every approx. 8 hours. It is carried out when a rapid assessment is required to familiarize ones self with a patient such as at shift change or at change of patient assignment.

what should you consider with infants when administering a head to toe?

A

The child may be fearful to new environment, Invasive procedures. Parents lap is preferred for the examination up to 3 years.

  • Provide a Toy during the exam.
  • Head to toe assessment.
  • Health history includes developmental data.
  • Evaluate the motor skills(fine and gross), posture, alignment of the legs, speech, vision and hearing.

A

because you may have problems breathing during your sleep and therefore have a severe case of COPD
