do not touch your patient or give meds until you do some type of assessment

how are you going to give it, when, how will you give meds. plan your cares out

the doing of giving meds, implementing things to help your patient

see how the medication is working, or not working. see what is going well and what is not. reassess and make changes accordingly
Prior to giving a medication, the nurse must gather pertinent information regarding the patient’s medical history, physical assessment, disease processes, and learning needs. Why is this important?
Knowing this information can impact the success of pharmacotherapy. One goal is to limit the number of adverse drug events, and this can help prevent cases from happening. Make sure we are not harming the patient. Checks if there are any contradictions.
what is an adverse effect?
undesirable sign or symptom associated with the use of a medication or treatment. Usually described in terms of intensity (mild, moderate, severe). An adverse effect warrants either lowering the dosage or discontinuing the drug. A serious adverse event is a threat of permanent disability to immediate risk of death. Not things that are typical.
what is a side effect?
an unintended nontherapeutic reaction to a drug in addition to the desired therapeutic effect (could be both positive or negative side effects). Perceived as tolerable. EX) side effect of antibiotic could be that you get diarrhea. An adverse effect would be an anaphylactic reaction where your airway closes up.
