A

Identifies the problem and writes it to the right of the diagram, in front of the fish (face)

Write the causes on branch (bones) “materials, environment, people, methods, machines, measurements.)

The team repeatedly investigates the problem along each branch, repeatedly asking “why does this happen?” to identify the root cause of each issue.

Review and discuss each cause in detail to identify what needs to change to solve the problem.
