Normal (N) Grade 5
Patient takes maximal resistance through full range of motion against gravity; can’t break at end of range
Good Plus (G+) Grade 4+
Patient takes nearly maximal resistance through full range of motion against gravity; can be broken at end of range
Good (G) Grade 4
Patient takes moderately high resistance through full range of motion against gravity; can be broken at end of range
Good Minus (G-) Grade 4-
Patient takes low moderate resistance through full range of motion against gravity; can be broken at end of range
Fair Plus (F+) Grade 3+
Patient can complete full range of motion against gravity; can hold end position against mild resistance
Fair (F) Grade 3
Patient can actively complete full range of motion against gravity; but can NOT take any resistance through or at the end of range
Fair Minus (F-) Grade 3-
Patient can actively complete at least half the range of motion against gravity; can NOT take any resistance through or at the end of range. Put them in gravity minimized
Poor Plus (P+) Grade 2+
Patient can complete range of motion and take some resistance in gravity minimized position; can take slight or minimal resistance
Poor (P) Grade 2
Patient can complete full range of motion in gravity minimized position; cannot take any resistance
Poor Minus (P-) Grade 2-
Patient can actively complete at least half the range of motion in gravity minimized position; cannot complete range
Trace Plus (T+) Grade 1+
Patient can produce minimal motion
Trace (T) Grade 1
Patient can produce no motion, but a contraction is detected visually or by palpation
Zero (0) Grade 0
Patient is unable to produce any contraction
