What constitutes “serious physical harm” and requires CAL/OSHA notification?
Inpatient hospitalization for purposes other than observation
The loss of any member of the body
Any serious degree of permanent disfigurement
Impairment sufficient to cause body part or organ function to reduce efficiency(2nd/3rd burns, crushing injury, respiratory illness and broken bones)

What documentation and notifications are needed if a radio is either removed or placed in to service?
Notify BC and Dispatch. Log book and Radio/Headset(intranet) repair request if taken out of service due to malfunction or damage.
What are the 2 main methods to decrease responses related to false alarms?
Billing for more than 3 responses within 90 days or
actively contacting the responsible party to resolve the cause of the false alarm.
What documentation must be done in NFIRS if false alarms is going to progress to billing?
A business name must be entered in to NFIRS
Who set occupancy load for a business?
Building Department
Who is notified for an overcrowding incident?
BC, if unable to resolve issue or habitual offender then the shift investigator will be asked to respond. OFD responsibility to enforce or provide violation notices.
What Stations have an extractor?
Stations 1,6,7, and 8
What will safety personnel be required to have in regards to the influenza vaccination?
Obtain the influenza vaccination
Provide documentation of outside vaccination
Sign the Seasonal Influenza Vaccine Declination Form
***If they have not received the vaccine they will be required to wear a surgical mask while within 6 feet of a pt during flu season as defined by the Department of Public Health
Per Department of Public Health the Flu season is ______________ and can be extended.
