Serious Injury
- Hospitalization for >48hr beginning within 7 days from date of injury
- Fracture of any bone (except simple fractures of fingers, toes, nose)
- Severe hemorrhage, nerve, muscle, or tendon damage
- Internal organ injury
- Second or Third degree burns or any burns affecting >5% of body
Substantial Damage
- Damage or failure that adversely affects the structural strength, performance, or flight characteristics of the aircraft
(prop strike is not substantial damage)

Occurrence other than accident associated with the operation of an aircraft that affects or could affect the safety of operations
