women w/ Migraine + Aura
Are a higher risk for stroke and should not use estrogen-containing contraceptives
Triptans
MOA, C/I
are selective for serotonin (5HT1) causing vasocontriction. Are 1st line for acute.
C/I: Stroke TIA, uncontrolled HTN, Ischemic heart disease or use within 24 hrs of another triptan.
Triptan sensation - Heavy Chest, Pressure in neck
Sumatriptan
Brand: Imitrex, STATdose, Onzetra, Zembrance Treximet ( +Naproxen)
Zolmitriptan
Zomig
Rizatriptan
Maxalt, MAxalt- MLT
Ergotamine Drugs
MOA, C/I, Use
MOA: Nonselective agonist of the serotonin receptor.
C/I in patients who did not receive benefit with triptans
Use: Next in line after triptan
Dihydroergotamine
Brand: BBW, C/I
D.H.E. 45, Migranal
BBW: AVOID CYP3A4 INHIBITORS
C/I: same as triptans
Ergotamine + Caffeine
Cafergot, Migergot
Prophylactic TXT
If pt has 2 or acute attacks/week or more than 3/month
Or if it decreases quality of life.
Beta Blockers
Use, which ones,
Propranolol (Inderal LA) (Lipophillic)
Timolol
Metoprolol (Lopressor, Toprol XL)
AEDs
Valproic Acid (BBW: Liver, pancreatits, Teratogenic)
Topiramate (Trokendi XR, Topiramate) weight loss, cognitive impairment, metabolic acidosis, paresthesia.
Rebound Headache
Limit acute TXT to 2 to 3 times per week. stop over used medication
