What is Mifepristone?
A Progesterone Receptor Modulator
How does Mifepristone Work?
- It blocks progesterone
- It stops the pregnancy from forming
- It causes the placenta and embryo to detach from the uterus
- It causes the cervix to dilate
- It sensitises the Uterus myometrium to prostaglandins so that the myometrium can contract more effectively.
How many hours after the ingestion of Mifepristone, is the maximal pharmacological effect?
36-48 hours
Why do we give Mifepristone?
It allows a lower dose of prostaglandin to be used, thus reducing side effects.
What is Misoprostol? (GyMiso)
It is a synthetic analogue of prostaglandin E1
How does Misoprostol work?
- It causes the contraction of the uterus.
- This leads to expulsion of the fetus.
- It also causes bleeding and cramps
How are Mifepristone and Misoprostol given?
They come in a pack.
- First give Mifepristone Line Pharma 200mg tablet.
- Then 36-48 Hours later take GyMiso 800mcgs (4 tablets).
What is a Medical Termination of Pregnancy (MTOP) like?
It is similar to a miscarriage.
What should you make sure you do before you give MS-2 Step and why?
An USS
- To confirm gestational age
- To exclude ectopic pregnancy
At how many days can MS 2-Step be used?
63 days.
