What does OPQRRRST stand for?
-Onset, provocation, quality (describe pain), region, radiation, relief, severity (0-10), and timing.
What questions can you ask for OPQRRRRST?
-O- When did the pain start? What were you doing when the pain started?
-P- Does anything make the pain worse? Does lifting, twisting, standing, walking make the pain worse? Does any medication relieve the pain? Do certain positions make the pain worse or better?
-Q- Can you describe the pain for me? Crushing, shooting, sharp, burning, aching, dull (aching), or throbbing.
-R- Where is the pain?
-R- Does the pain stay in the “painful region”or does it move/radiate to other places?
-S- Rate your pain out of 10? Zero represent no pain and ten representing the worst pain.
-T- When did the pain start?
What are the doses for naloxone?
-1st dose - 0.4 mg
-2nd dose - 0.4 mg
-3rd dose - 0.8 mg
-4th dose - 2.0 mg
What is a sign of improvement for an overdose after giving naloxone? What should you check before giving each dose?
-Respiratory rate increases greater than 10, and/or LOC increases.
-Assess LOC and respiratory status prior to each subsequent dose of Naloxone.
When should transport for an overdoes call?
-After the first dose of Naloxone.
When do you give oral glucose? What are the doses? What is the maximum dose?
-When BGL is equal or less than 4.0 mmol/L administer 1/2 a tube of oral glucose.
-If the patient is able to follow commands and maintain airway administer orally. If they’re unable to administer to the patient’s buccaneers pouch while the patient is 3/4 prone.
-If no improvements within 5 minutes, repeat BGL test and if equal or less than give oral glucose every 5 minutes to a max of 50 grams.
What are the contraindications for Aspirin?
-A- Acute Bleeding
-S- Sensitivity/Allergy to Aspirin
-A- Asthma
-O- Overdose
-K- Kids (not for pediatrics)
What are the contraindications for nitroglycerin?
-Allergy or Sensitivity to Nitrates.
-BP must be greater than 110.
-Erectile dysfunction medications in the past 24 hours.
What are the doses of nitroglycerin? How often should they be given and where? What is the max dose?
-0.4 mg of Nitroglycerin under the tongue and administer when the patient is in the cot. Transport immediately after 1st dose of Nitroglycerin.
-Every 3 minutes until you reach max 3 doses or pain goes to 0.
-If you reach max dose of nitroglycerin then consider Entonox.
What is the max dose of ASA?
-162 mg.
-If the patient has not taken a full dose within 12 hours top up to 162mg. If they haven’t taken a full dose in the past 12 hours, administer full dose.
What are the 6 rights of medication administration?
-Right Drug, right date, right documentation, right dose, right patient, and right route.
What are the contraindications for naloxone?
-Allergy to Naloxone, ask bystanders/family/friends.
-Naloxone should not be given to a neonate.
What are the contraindications for entonox?
-P- Pneumothorax
-A- Air embolism.
-I- Inhalation injury.
-N- Nitroglycerin in the last 5 minutes
-D- Decompression sickness. Scuba diving recently?
-I- Inability to comply.
-E- Enclosed space.
What should do you do before administering entonox?
-Inverting the tank 3 times.
How do you explain the use entonox to the patient?
-When you feel like you’re in pain, bring the tube to your mouth and breath in the gas.
