Microbio- zoonoses + prion disease Flashcards
Definition of zoonoses?
Infections acquired from vertebrate animals
3 infections from rats?
Rabies, plague, Hantan viruses
5 infections from cats
Bartonellosis, Salmonella, Rabies, Q-fever, Toxoplasmosis
5 infections from dogs
Hydatid disease, salmonella, rabies, leptospirosis, ringworm
Small ruminants - 3 infections?
Brucellosis, anthrax, Listeria
5 infections from cattle?
Toxoplasmosis, bovine TB, E coli 0157, anthrax, brucellosis
4 infections from birds
Psittacosis
Influenza A
West Nile virus
cryptococcus
3 infections - Water sports associated
Leptospirosis
Hep A
Giardia
Brucella - what kinda bacteria is it? How is ti transmitted? Which animals?
Gram -ve aerobic bacilli
Facultative intracellular
Transmitted by consumption of untreated dairy.
RUMINANTS + PIGS
Undulant fever, spinal tenderness, lymphadenopathy, hepatosplenomegaly +consumption of shit dairy
Brucellosis
Serology results +ve for brucellosis?
anti-O-polysaccharide antibody
Treatment of brucellosis
Tetracycline or doxycycline, combined with streptomycin
Negri bodies
Rabies
Rabies - serology? Treatment?
ELISA for specific IgM, or RT-PCR
Rabies IgG post-exposure
Plague - what is the bacteria involved?
How is it transmitted?
Bacteria = Yersinia Pestis
In rats, transmitted by fleas
Ingestion of unpasteurised milk –> fever which comes and goes + back pain.
Dx? Ix? Tx?
Dx: Brucellosis
Ix: - anti-O-polysaccharide antibody
Tx: - Tetracycline + streptomycin
Which zoonosis is excreted in dog/rat urine?
Leptospirosis
Painless round black lesions + rim of oedema
Cutaneous anthrax
Rash seen in Lyme disease?
Erythema chronicum migrans
Lyme disease: early stage of illness? Late stage of illness?
Early stage: flu-like, nerve palsies, arthritis, carditis
Late stage: encephalopathy, arthritis
Tx of lyme disease?
2-3 weeks of doxy
4 types of leishmania disease?
Cutaneous
Diffuse cutaenous
Visceral (Kala-Azar)
Mucocutaneous
Diff between lesions seen in diffuse cutaneous vs mucocutaneous leishmaniasis
Diffuse cutaneous: NO ULCERS
Mucocutaneous: ulcers in mouth/nose (mucus membranes)
Vector of leishmaniasis?
Sandfly
Prion disease - on which chromosome is the prion protein gene? which codon is important?
Chromosome 20
Codon 129
which polymorphism is most associated with prion disease?
MM on codon 129
Most common form of prion disease?
Sporadic CJD
Disease course of sporadic CJD?
Rapidly progressive ataxia + amnesia + myoclonic jerks + LMN signs.
Death in 6 months
Post mortem findings in sporadic CJD - give 2
- PrP amyloid plaques
- Spongiform vacuolation
In what form of CJD is tonsillar biopsy super useful?
Variant CJD
- Tonsillar biopsy is 100% sensitive + specific
MRI shows posterior thalamus is highlighted. Which form of CJD is most likely?
variant CJD
What polymorphism is seen in all cases of variant CJD?
MM on codon 129
Typical course of symptoms in variant CJD?
Initially PSYCH: anxiety, paranoia, hallucinations
Then, neuro: ataxia, myoclonus
WTF is Kuru
Papua New Guinea - exposure to CJD from cannibalistic feasts
- Progressive cerebellar syndrome following 45 year incubation period
Autosomal dominant inherited condition where insomnia and paranoia leads to hallucinations and weight loss and mutism
Fatal familial insomnia
