L Lactate - Type A and B
L-lactate is the end product of anaerobic glycolysis.
There are two major clinical settings in which lactic acidosis occurs
Type A’ - Increased lactic acid production occurs when cellular respiration is abnormal, because of either a lack of oxygen in the tissues –> hypoxia driven
This type is the most common.
Type B - a metabolic abnormality or drug/toxin induced. Examples include -
Drugs - salbutamol and metformin
metabolic disorders - diabetes mellitus, hepatic disease and neoplasia.
Congenital metabolic disorders include type I glycogen storage disease.
The most common cause in clinical practice is type A lactic acidosis, occurring in septic or cardiogenic shock
