Metabolic acidosis Flashcards
What characterises metabolic acidosis?
Low pH and low bicarbonate < 22 mmol/L
PaCO2 in metabolic acidosis?
Will be low as the compensatory mechanism is to hyperventilate
Most important thing to remember =
Low bicarbonate < 22 mmol/L in metabolic acidosis
What causes normal anion gap acidosis?
Loss of bicarbonate e.g. diarrhoea or from the kidney and Cl will rise to provide electroneutrality
What is the mechanism behind a raised anion gap acidosis?
Gain of acid resulting in removal of bicarbonate due to formation of carbonic acid and increase in unmeasured anions to provide electroneutrality
What does the mnemonic GOLDMARK signify?
All the causes of raised anion gap metabolic acidosis
G in GOLDMARK?
Glycols (ethylene, propylene)
O in GOLDMARK?
Oxyproline
L in GOLDMARK?
Lactate
D in GOLDMARK?
D-lactate
M in GOLDMARK?
Methanol
A in GOLDMARK?
Aspirin
R in GOLDMARK?
Renal failure
K in GOLDMARK?
Ketoacidosis
What causes lactic acidosis?
Anaerobic respiration which occurs in shock (hypovolaemic, cariogenic, septic, anaphylactic)
