A

Major surgery

ADH secretion

  • bronchogenic carcinoma
  • adenocarcinoma
  • lymphoma

Drugs

  • antidepressants: SSRI, TCA
  • antipsychotics: haloperidol
  • chemo
  • amiodarone, ciprofloxacin

CNS disorders

Hormone deficiency

  • adrenal insufficiency
  • hypothyroidism

Others

  • GBS
  • HIV (pjp)

Pulmonary disorders

  • pneumonia
  • TB
  • lung abscess

Urinary Na > serum Na
Urinary Na > 20mmol
Euvolaemia
Normal TFTs and adrenal function 
No diuretic use
Normal cardiac, hepatic, renal function

Causes of hyponatraemia

A

Hypovolaemia

  • excessive loss
  • urinary Na < 20mmol extrarenal loss due to sweating, vomiting, diarrhoea, pancreatitis
  • urinary Na > 20mmol renal loss thiazides, osmotic diuresis, RTA or CKD

Euvolaemia

  • SIADH (diagnosis of exclusion)
  • beer potomania
  • psychogenic polydipsia
  • drugs
  • H2O intoxication

Hypervolaemia

  • failures
  • urinary Na > 20mmol renal failure
  • urinary Na < 20mmol cirrhosis, cardiac failure, nephrotic syndrome
