How would one go about giving smoking cessation advice?
Ask - ask about smoking and their habits
Advise - on the risks of smoking and the benefits of cessation
Assess - assess their motivation to quit smoking and determine willingness to quit
Assist - aide in quitting, point them to smoking cessation areas
Arrange - arrange a follow up appt and ask them again at this
What is schizophrenia? What are some symptoms?
Psychiatric disorder
Positive symptoms that alter behaviour or thoughts
- hallucinations or delusions
Negative symptoms
- withdrawal from life and social activity around them
- no interest and flat emotions
What are some side fx of old anti-psychotic medication?
Pyramidal tremor causing involuntary movement that may not get better when medication is stopped
What medication might someone with schizophrenia be on? What are some considerations?
Clozapine - not first line drug
Clozapine decreases neutrophil Level, so need full blood count and neutrophil level to be over 1
Causes hypersalivation
Stopping smoking on clozapine causes increase in the effects of the drug greatly, so be very careful as stopping smoking first might not be the best benefit
What side fx might a salbutamol inhaler give? How remove this?
Candida
Use a spacer or rinse your mouth after use
How might schizophrenia be managed?
Multi disciplinary team
Therapy and councilling
Social work
Teaching life skills
Dental and physician
How can you assess capacity?
Act - did the pt make their own appt or seek help
Make - can pt make their own informed decision
Communicate - can pt communicate effectively
Understand - can patient understand their treatment - chunk and check
Retain - can patient retain the information given and remember the appt?
When can one make a decision on behalf of someone else?
When…
- There is a benefit to pt
- Minimum Necessary intervention
- Take into account wishes of adult
- Consult relevant others
- Encourage adult to exercise residual capacity
What are some red flags of a mouth ulcer?
Persists >weeks without getting better
Rolled borders
It is firm and not soft
It is in high risk factor patients
When there is non obvious cause e.g. traum
What nutritional deficiency may cause ulceration?
Low:
Folate
B12
Iron
When is steroid cover indicated in dentistry?
If patient is on >5mg prednisone or equivalent for >4 weeks
Should have steroid cover
Oral prednisolone 20-25mg / day for 3-5 days starting day before procedure and continuing for 1-2 days after
