Melanoma left over Flashcards
What is the definition of a melanoma?
-Malignant neoplasm of melanocytes (melanin producing cells) mainly arising in the skin
Other sites: nasal cavities, retina, GI mucosa
Difference between melanoma and SCC?
-SCC superficial layers, melanoma deeper layer (granulosum)
-continuous vs intermittent sun exposure
-Arises in sun exposed areas vs anywhere
-Appears as red bump/scaly patch/non healing sore vs irregular mole
-Better vs poorer prognosis
-Metastasis less vs more common
Difference between BCC and SCC
-Spreads less vs more commonly
-Stratum basale vs more superfiical layers
-more vs less common
-Raised pearly edges vs thickened exophytic growth with necrotic core
What to do to ensure adequate margins
Frozen secion
-Moh’s surgery
Post excision the patient developed regional lymphadenopathy, management?
-FNAC
-Staging CT
How would you manage PE?
-CTPA
-SC LMWH if haemodynamically stable
-Thrombolysis or embolectomy for massive PE
What is epitheloid melanoma?
-Melanoma cells can be categorised into 2 major cell types: epithelioid and spindle cell
-Epithelioid are large and round with abundant eosinophilic cytoplasm, prominent vesicular nuclei and large nucleoli
-They most commonly arise in superficial spreading and nodular melanomas
What is a satellyte lesion?
-Form of local spread of malignant melanoma by contiguity and continuity leading to pigment spreading to surrounding area. FOund within 2cm of primary tumour
