what medications do you use for fluid resuscitation?
TXA 1 gm 100 ml NaCl bag- 10 min Hextend 500 ml (up to 2 doses) BOLUS Lactated Ringers ( last resort) 1000 ml
what meds do you use for pain management and antibiotics ORALLY?
Tylenol- 650 mg tablets (2 tablets, 8 hour PO)
myloxican/Mobic- 15 mg ( once a day)
Moxi- 400 mg dose ( 1 tablet once a day)
what meds do you use for pain management when they’re not able to swallow?
NON IV: OTFC 800 mcg (lozenge in the mouth)
Ertapenden (IM) 1gm mixed with 3.2cc of lidocaine without epi
Ketamine (IM) 50 mg dose, check on them 30 min
NOT GIVE OTFC AND KETAMINE AT SAME TIME
IV Access: Ketamine 20 mg dose, repeat dosage every 20 PRN for pain (IV/IO)- slow iv
Ertapenem: 1 gm with 10 ml NaCl, shake, mix with 50 ml Nacl over 30 infuse time
when do you give ketamine?
when the pt is in shock or respiratory distress, or risk for it, if in moderate to severe pain
when do you give iv morphine?
alt. to otfc, 5mg iv/io. only give if pt is not in risk for resp. distress or shock, unconscious, if moderate to severe pain IF YOU HAVE IV/IO ACCESS
What is for nausea and vomiting?
Odanzetron (ODT- oral dissolvable table, io/iv/im) 4 mg every 8 hours AS NEEDED. DO NOT GIVE MORE THAN 8 mg. can give after 15 min if still feeling pukey but only one more
What is the drug reversal?
Naloxone (narcan) .4 mg iv/im for opiods
what do you do to decrease intercranial pressure for a tbi?
3-5% of hypertonic NaCl Bolus 250cc, elevate head to 30 degrees and hyperventilate pt
