Dobutamine
Increase heart contractility and increases heart rate
Lowers PVR peripheral vascular resistance
2.5-20 mcg/kg/min
For CHF, shock (cardio, septic)
Norepinephrine
Low dose beta/ high alpha For shock (cardio, septic), GIB
If Levo infiltrates treat with
Regitine
Epinephrine
Low dose beta/high alpha
.005-.02 beta increase HR, decrease SVR
High doses increase SVR, BP, renal constriction
For cardiac arrest, shock, anaphylaxis, asthma
Phenylephrine
Pure alpha increase BP, PAP, heart and
renal constriction
40-60 mcg/min
Vasopressin
Alpha vasoconstrictor
ACLS 40 units, IV .04 units/min
Milrinone
Phosphodiasterase inhibitor
Increases CO
for acute CHF, cardiomyopathy
Nipride
Vasodilator, decrease SVR
for severe Heart Failure, M regurg, HTN crisis
0.25-10 mcg/kg/min
S/E: cyanide toxicity
Nesiritide (Natrecor)
BNP, vasodilation, natriuresis,
Not compatible with lasix, bumex, insulin, heparin
Nicardipine (Cardene)
CCB, lower BP and after load
0.1 mg/hr
Beta Blockers
Decrease heart contractility
Squeeze blood out slower
Calcium Channel Blocker
Slows electrical conduction, dilates to decrease SVR
Alpha and beta drugs
AC -BD
Alpha constricts
Beta dilates
