What factors can impact the risk for food/nutrient and drug interactions?
Age, gender, medical history, body composition, nutritional status, and number/kinds of other medications

Increase or decrease appetite, altered taste, drug mouth, N/V, diarrhea, constipation, hyper or hypo glycemia, altered fat metabolism/absorption, sodium/potassium/phosphorus alterations, and loss of calcium

YES! It protects you and the facility you work out so be sure to be clear and thorough

Name some mechanics of good documentation
Write legibly, date and time all notes, write in blue or black ink (depends on facility preference), never use correction fluid, do not skip lines on the progress notes, and check your spelling!
