Serotonin receptor antagonist, used in serotonin syndrome

Hyperthyroidism
Inhibits conversion of T4 to T3 in extrathyroidal tissue
Advantageous in neonatal thyrotoxicosis

10x more potent than Propylthiouracil
Inhibit incorporation of trapped inorganic iodide into organic compounds
Clinical response 3-6 weeks, control 3-4 months
Remission 25% by 2 years
If relapse will happen usually within 3 months, always by 6 months
Earlier remission: >13 years old, boys, inc BMI, small goitres, and modestly elevated T3

Arrhythmias, bradycardia, hypotension –> continuous bp/ECG monitoring
Phenobarbital side effects
Sedation, resp depression, hypotension

Intractable infant seizures may be caused by an undiagnosed metabolic disorder such as Pyroxidine-dependent epilepsy
