Adverse drug event
Injuries resulting from the use of a drug, which include harm caused by the drug (ADRs and
overdoses) e.g. patient on lisinopril normal dose who develops angioedema requiring tracheostomy
Medication errors
Mishaps that occur during reconciling, prescribing/ordering, transcribing, dispensing, administering, or monitoring a drug
Adverse drug reaction
any response to a drug which noxious and unintended during the use of normal doses in treatment or prophylaxis
Preventable Adverse drug events
a medication error that reached the patient and resulted in harm e.g. Entering a high dose of drug in the infusion pump that resulted in patient having an organ dysfunction

when patient is given the wrong drug : e.g. Cefazolin instead of cefepime which will result in worsening infection and potential injury to patient
