Dextrose

A

Class: Carbohydrate, Antihypoglycemic

Dosage: 12.5-25 grams of a 50% solution slow IV push

Indications: Hypoglycemia, Altered level of consciousness, Coma of unknown origin, Seizure of unknown origin, Status Epilepticus

Contraindications: Intracranial hemorrhage

Valium

A

Class: Benzodiazepine, Long acting sedative-hypnotic, Anticonvulsant, schedule 4 drug

Dosage: Seizure activity: 5-10 mg IV every 10-15 min PRN (5 mg over 5 min) to a max of 30 mg

Indications: Acute anxiety states and agitation, Acute alcohol withdrawal, Muscle relaxant, Seizure activity, Sedation for medical procedures (intubation, vented patients, cardioversion), maybe helpful in acute symptomatic

Contraindications: Hypersensitivity, Narrow angle glaucoma, myasthenia gravis, respiratory insufficiency, coma, head injury

Atrovent

A

Class: Anticholinergic, Brochodilator

Dosage: 250-500 mcg inhalation w/ neb every 20 min max of 3 times

Indications: Persistent Bronchospasm, COPD exacerbation

Contraindications: Hypersensitivity to: Ipratropium, atropine, alkaloid, soybean, peanuts

A

Class: non steroidal anti inflammatory (NSAID)

Dosage: Adult: 30-60 mg IM, Pediatric: Not Recommended

Indications: Short term management of moderate to severe pain

Contraindications: Allergy to salicylates or other NSAIDs, Asthma, bleeding disorders, especially GI related, renal failure

A

Class: Hyperglycemic agent, Pancreatic hormone, Insulin antagonist

Dosage: Hypoglycemia: 0.5-1 mg may repeat in 7-10 min, calcium channel blocker or beta blocker overdose: 3-10 mg IV slowly over 3-5 min initially, followed by a 3-5 mg/hr infusion as necessary

Indications: Altered level of consciousness when hypoglycemia is suspected, may be used as a reversal agent in beta blocker and calcium channel blocker overdoses

Contraindications: Hyperglycemia, Hypersensitivity

Atropine

A

Class: Anticholinergic

Dosage: Unstable Bradycardia: 0.5 mg IV/IO every 3-5 min as needed to a max of 3mg
Organophosphate poisoning: Large doses maybe needed 2-4 mg or higher

Indications: Hemodynamically unstable bradycardia, Organophosphate poisoning, Nerve agent exposure, RSI in pedis, Beta blocker or calcium channel blocker overdose

Contraindications: Tachycardia, Hypersensitivity, Unstable cardiovascular status in acute hemorrhage w/ myocardial ischemia, narrow angle glaucoma

A

Class: Sympathomimetic

Dosage: Dilute 5 ml of solution in 5 ml saline in Nebulizer

Indications: Bronchial asthma, Prevention of Bronchospasm, Croup, Laryngotracheobronchitis, Laryngeal edema

Contraindications: Hypertension, underlying cardio vascular disease, Epiglottitis

Class: Sympathomimetic

Dosage: MILD ALLERGIC REACTIONS: 0.3-0.5 mg 1:10000 SC
ANAPHYLAXIS: 1 mg IV/IO over 5 min
CARDIAC AREEST: 1 mg IV/IO every 3-5 min followed by a 20 mL flush and elevate arm
CONTINUOUS INFUSION: add 1 mg to 250 mL of saline starting at 1 mcg/min 2-10 mcg/min

Indications: cardiac arrest, symptomatic bradycardia as an alternative to dopamine, Severe hypotension secondary to bradycardia when atropine and transcutaneous pacing are unsuccessful, allergic reaction, anaphylaxis, asthma

Contraindications: hypertension, hypothermia, pulmonary edema, myocardial ischemia, Hypovolemic shock

Class: calcium channel blocker, antiarrhythmic

Dosage: 0.25 mg/kg IV over 2 min after 15 min rebolus 0.35 mg/kg IV over 2 min

Indications: controls rapid ventricular rates due to: a fib, A flutter, reentry SVT

Contraindications: Hypotension, sick sinus syndrome, second or third degree heart block, cardiogenic shock, wide complex tachycardia, poison or drug induced tachycardia

Dosage: V fib and pulse less V tach: 300 mg IV,IO followed by one time 150 mg in 3 to 5 min
Wide complex tachycardia: 150 mg over 10 min

Indications: Ventricular Arrhythmias, second line for atrial arrhythmias

Contraindications: Sick sinus syndrome, second and third degree HB, Cardiogenic Shock, Syncope caused by bradycardia, sensitivity to benzoyl alcohol and iodine

Contraindications: Sick sinus syndrome, second or third degree HB, poison or drug induced tach
