SALT

A
  • Swallow assessment to ensure they can swallow foods safely.
  • Swallow and speech exercises for dysarthria (motor), dysphagia
  • provide communication aids or means of communication for patient to physio or other MDT/ fam
  • physio can teach positioning for optimum swallow, comfort, use communication aids
    Eg. in PD the flexed posture reduces ability to swallow so positioning could help

A
  • Home assessment, access to bathroom, what is upstairs
  • Provide assistive equipment to aid ADLs and reduce effort = independence
  • Teach ECTs alongside physio to make sure they are managing ADL: eg. pacing, prioritising and planning
  • Tell physio if need to work on anything specific for ADLs or function independence (eg: amount of stairs in house or mobility ex if poor accessibility)
    PT ensures exercise capacity is carried over into ADLs = independence

Pharmacists

A
  • Prescribing medication for symptoms which may hinder px function or ability to perform discharge ex.
    (E.g. pain medication advise)
  • Can organise pill boxes to be delivered to patients doors if they don’t have the ability/ support to collect medication themselves
  • this also helps to organise pills and when they should be taken (cognitive deficit)
  • Pharmacists can advise physios and patients when to take their medication and on any side effects which may hinder their ex, to promote the optimal effects from Medication to improve PA (eg: AM/ PM, straight after taking meds)

Dietician

A
  • Patients are consuming safe food, if poor swallow or tongue tremor, does food need to be liquified or pureed
  • ensure they are consuming nutrition to help with bodily and brain function, fatigue and energy to complete PA and physio
  • Ensure consumption is fitting for their energy expenditure (often sedentary) avoid wight gain etc
  • discuss with patients and physio benefits of good diet and PA for their neurological condition to avoid joint pain and improve exercise tolerance
