AD
Alzheimer’s disease: Most common type of dementia, progressive disorder with memory loss and increased physical impairments over time. Avoid arguing, offer comfort, distraction techniques, follow routine, identify unmet needs
CAD
Coronary artery disease: Hardened narrow blood vessels, decreased oxygen and blood supply to heart. Atherosclerosis-plaque on artery walls made up of ft and cholesterol. Can cause angina, heart attack, irregular heartbeats, sudden death
CHF
Congestive heart failure: dietary statue, NAS, fluids, daily weigh ins, vitals, SOB, dyspnea, activity intolerance
Contact precautions
Contact isolation precautions: wash hands before and after, wear gown and gloves. I.E. patient with C. Diff (clostridium difficile)
COPD
Chronic obstructive pulmonary disease (emphysema, bronchitis) is primarily caused by smoking. IMPORTANT FOR CARE: positioning, dyspnea, pulse oxygen, O2 needs, nebulizer, monitor vitals (esp R), limited physical exertion, don’t rush care, avoid restrictive clothing, pursed lip breathing
CVA
Cerebral vascular accident (stroke) IMPORTANT FOR CARE: ROM, encourage independence, supervise feeding, fall prevention, assistive devices, possible dysphagia, sensory disturbances, memory, emotions, difficulty communicating
Dementia
Umbrella term for loss of brain functioning and neuron death. Loss of cognitive functioning that interferes with daily life and activities
DM2
Diabetes Mellitus type 2 IMPORTANT FOR CARE: signs and symptoms of hypo-hyperglycemia, regular meals, snacks, activity, daily skin/feet checks, wear socks, DO NOT cut nails
ESRD
End stage renal disease (kidney failure, kidney transplant, dialysis: removing waste from blood) IMPORTANT FOR CARE: fluid retention, fluid restriction, daily weigh ins, low protein/low K+/low sodium diet
GI
Gastrointestinal (includes mouth, esophagus, stomach, small and large intestines, rectum, and anus)
Hip Fx
Hip fracture IMPORTANT FOR CARE: hip precautions, fall prevention, monitor vitals, signs of infection, swelling, pain, dressings, ice pack, bathing precautions
HTN
Hypertension (high blood pressure) IMPORTANT FOR CARE: monitor B/P, NAS, compression stockings, monitor for S/Sx of orthostatic hypotension possibly due to medications
MS
Multiple sclerosis (muscle weakness, possible speech impairment, relapsing symptoms, sensitive to extremes in temp)
PA or PNA
Pneumonia (fever, chills, cough) IMPORTANT FOR CARE: encourage hydration and C/DB (coughing and deep breathing)
PD
Parkinson’s disease: progressive disorder where movement deteriorates, tremors, stooped posture, flat facial effect
RA
Rheumatoid arthritis: painful, adaptive devices, fall prevention, safety protocols
RIS

Traumatic brain injury (occurs from violent injury to the brain)
URI
Upper respiratory infection IMPORTANT FOR CARE: droplet precautions, gown, gloves, mask, face shield
UTI
Urinary tract infection IMPORTANT FOR CARE: encourage hydration, report frequency, burning, urgency, flank pain, fever, color of urine, promote good perineal hygiene
