Euvolemic and pre-BP <140/90
Post-BP <130-80 (adult)
Lower of 90% of normal for age/ht/wt or < 130/80 (pediatric)
Avoid UFR >/= 13 mL/kg/hr
What is the HD adequacy standard?
> /= 1.2 kt/v, URR >/= 65
Measured monthly
What is the PD adequacy standard for adults?

Measure 1st month and q 4 months afterward

Calcium unadjusted <10.2 mg/dL 3 month rolling avg (q month)
Phos 3.5-5.5 mg/dL (q month)
PTH no standard ranges (q 3 months)
What are the anemia standards for pts not on ESA therapy?
No safe UL established
What are the anemia standards for pts not on ESA therapy (adult and pediatric)?
No safe UL established
What are the anemia standards for pts on ESA therapy (adult and pediatric)?
Initiate ESA hgb <10 mg/dL
Interrupt or decrease dose near or > 11 mg/dL
Give lowest dose of ESA to avoid transfusion especially in transplant candidates
Consider pt preference
What are the iron store standards for adults and pediatrics?
Monitor TSAT and Ferritin routinely
>20% TSAT (HD/PD)
>200 ng/mL (HD), >100 ng/mL (PD)
< 500 ng/dL or evaluate if needed
