What are the core symptoms for mania?
Think MANIC
- Mood: elated, excited, enthusiastic
- Activity increased
- Naughty: spending, alcohol, drugs, sex
- Insomnia
- Confidence increased
What questions would you ask to elicit symptoms suggestive of a manic episode?
- Mood: how wold you describe your mood?
- Increased activity: how would you describe your energy levels?
- Sleep: how is your sleep, when do you go to sleep? Would you say your need for sleep has changed recently?
- Increased libido: has your sex interest changed recently?
- Disinhibition: Would you describe yourself as impulsive?
- Self-worth: how do you see yourself compared to other people?
- Concentration: do you have any difficulties concentrating on things like reading books or watching TV?
What is important to ask for mania?
INSIGHT!
Do they have insight into their disease?
“Do you think everything is okay right now?” “What do you think is causing your change in behaviour?”
What are differential diagnoses for mania?
- Psychosis: “do you have any special powers” “Do you hear things that cannot be explained?”
- Physical causes: drugs, health problems (frontal love disease)
What is the definition of a manic episode?
Distinct period of persistently elevated, expansive and irritable mood. Most last for at least one week and features include:
- Increased self esteem and grandiosity
- Decreased need for sleep
- Talkative
- Flight of ideas and racing thoughts
- Distractibility
- Increased goal-directed activity and psychomotor agitation
- Excessive involvement in pleasurable activities
How would you distinguish mania from hypomania?
Hypomania typically lasts a couple days and does not require hospitalisation. Symptoms are less extreme and often do not impair an individual’s ability to function in. everyday life as much
