What to consider if pt says feels loose?
How long has it been like this - is fatigue issue?
What is peri-implantitis?
Periodontal disease of the implant
How tx peri-implantitis?
PMPR to remove sub-gingival deposits - pt maintenance and OH crucial
How to examine implants?
Take thorough hx Look at soft tissue and implant Check mobility Check pocketing Check pus - palpate ridge
Ideal way to take radiograph of tooth?
Take perpendicular to ensure threads in focus
What is basic structure of implant?
Implant/ fixcture Abutment screw to hold core in place Abutment - like post and core Prosthetic screw Crown that fits on abutment
What do pt expect post-surgery?
Often expect full tooth
Need to understand 3 elements and crown will be fitted later
Adv of screw > cement
Screw allows crown to be placed w/o cement - avoids excess which can cause peri-implantitis
What are some mechanical/ material failures?
Fractures porcelain
Screw loosen
Worn/ broken clips
If pt has bruxism what should consider?
Reinforced metal surface
What are biological issues with implants?
Gingival issues: inflammation, recession, bleeding, bone loss, peri-implantitis
Failure to integrate
Damage nerve
What can be aesthetic issues with implants?
Poor shape, colour or position
Recession - metal showing/ black triangle
What can be functional issues with implants?
Can’t chew
Lisp
Pain/ discomfort
Loss proprioception
