Benzodiazepines are 1st line treatment - usually IV lorazepam is used. Buccal midazolam is good when IV access has not been obtained.

A

If tonic-clonic seizure is prolonged or recurrent emergency buccal midazolam may be given 1st line in the community according to pre-arranged protocol.

  • 1st line - recommended mono therapy and switch AED if not working - carbamazepine + lamotrigine
  • 2nd line - combination therapy if mono therapy hasn’t led to seizure free
