What is the NMDA receptor antagonist and when would you use it?
Memantine
Alzhemers where acetylcholineesterase inhibitors contraindicated or in severe disease
How does memantine work?
Reduced glutamate excitotoxic effects
When the cells die they release glutamate which causes calcium influx in adjacent cells and their resultant death - like a chain reaction
Memantine stops this
What are the side effects of memantine?
Cons
HTN
Dyspnoea
Headache
DIzziness
What is the treatment for vascular dementia
Modify risk factors - Aspirin
When would you use anti parkinsonian medication?
Lewy body dementia
What do antipsychotics increase the risk of?
Stroke
What antipsychotic could be used?
Risperidone
Why is using antipsychotics controversial?
Increase risk of stroke and need to weigh up the risks/benefits
What other medications can be used for dementia?
SSRIS
