- Relation to history and symptoms
- Recap to rfv, symptoms -> examinations to investigate symptoms - Describe findings
- results of key investigations - Describe diagnosis
- Explain disease, cause, prognosis - Explain management options
- Describe all options, and advise plan - Explain next steps
- Follow up appointment
- Pharmacological treatment

Follow up appointment:
• Supplementary eye exam
• New symptoms
• Repeat procedures
• Follow-up appointment
- Assess if disease is resolving, or responding to treatment
- usually done at clinical appropriate interval:
Types of referral:
• Routine: 3-9month waiting list
- Ocular disease unlikely to significantly or irreversibly progress in this time
• Urgent: Next few days, up to one week
• Emergency
Electronic referral:
• Routine referral through online system
• Avoids delayed/lost referrals
• Clearly legible
• Enables file attachment (e.g. fundus photograph)
• Referrals not read immediately:
Emergency/Urgent Referrals: Telephone call still required
