What are the three types of breech presentation?
Complete- both thighs and legs flexed
Frank- thigh flexed on abdomen and both legs extended at knee
Footling- one of both legs extended below the level of the buttock
What are the fetal causes of breech?
Congenital anomalies e.g. Hydrocephalus Prematurity Macrosomia Multiple pregnancy IUGR and IUFD
What are the maternal causes of breech?
Pelvis abnormalities (tumours, contracted pelvis)
Abnormal placentation
Uterine abnormalities (fibroids, septate)
Multiparty
Poly/ oligo- hydramnios

Clinical exam- palpation and ballottment,

What are the complications of breech delivery?
Birth asphyxia - cord prolapse and cord compression Birth injuries (-fetus): brachial palsy, long bone fractures, tears in tentorium cerebelli, spinal cord disruptions
What are the indications for C/S in breech?
EFW 3700g Contracted pelvis Footling breech Deflexed head Any other obstetric complication
What is a compound presentation?
Prolapse of a fetal extremity into the lower uterine segment alongside the presenting part
What causes a compound presentation?
An ill-fitting resenting part predisposes: Prematurity CPD Multiple pregnancies Polyhydramnios Abnormal lie (transverse)
How is compound presentation managed?
Confirm diagnosis Establish fetal viability If previable, allow vaginal delivery If viable, outcome is better with C/S Umbilical cord prolapse is a risk in compound presentation
What is the prognosis for compound presentation?
25% perinatal mortality
Main causes- prematurity, cord prolapse, traumatic vaginal delivery
