Epididymo-orchitises
Epidemiology:
Kids: congenital GU abnormality → gram neg rods (e coli)
Sexually active young men: gonorrhoea and C trachomatis
Older than 35: pseudomonas
Gonorrhoea: suppurative if severe → abscesses
Mumps: systemic infection, if in school aged kids tend to not have testicular involvement, however if post pubertal, orchitis occurs in 20-30%
Orchitis typically develops 1 week after onset of swelling of parotid glands
TB: always begins in epididymis, may spread to testis. Caseating granulomatous inflammation
Syphillis: may be affected in congenital and acquired syphilis
testis is always involved first, and often epididymis is spared all together (although may be involved)
