What should be applied on the skin before outdoor activities?
DEET
30% - ok for children as young as 2 months and up.
how long is 30% DEET effective for?
4 to 6 hrs
how long is 15% DEET effective for?
5 hrs
how long is 10% DEET effective for?
3 hrs
how long is 5% DEET effective for?
2 hrs - safe in 6 months to 2 yrs old
What are the risks of ingesting DEET?
can lead to seizure, hypotension, angioedema and death
should DEET be applied before or after sunscreen?
AFTER. This would allow sunscreen to penetrate well.
What are the pathogens in MALARIA?
P. falciparum
P. vivax
P. ovale
Which antimalarial is contraindicated in seizure, psychosis, depression and anxiety?
mefloquine
How is mefloquine taken?
Start 1-2 weeks before patient leaves, then once a week during the trip, then for 4 weeks after the trip.
Which antimalarials are safe in pregnancy?
Chloroquine ( in chloroquine-sensitive areas), but in resistant areas, mefloquine is safer after the 1st trimester of pregnancy, can be used in children < 5 years old
In mefloquine-resistant areas, what are the safe chemoprophylaxis agents in pregnant women?
None.
only chloroquine is safe in pregnancy.
What side effects of mefloquine would require discontinuation?
Severe: seizures, psychosis (early on).
Less severe: anxiety, depression, irritability, nightmares. more common in women especially.
What are the neurological warnings of Mefloquine?
dizziness, loss of balance, tinnitus (can become permanent)
