Common symptoms?
large vesicles, reddish spots clustered around nose, lips and flexures, blisters ooze and burst, forming a yellow honey-color crust
bacterial cause of non-bolus impetigo?
S. pyogenes
first line tx for non-bolus impetigo
topicals - mupirocin or fusidic acid x 7-10 days
bacterial cause of bolus impetigo (widespread)
S. aureus
1st line for bolus impetigo?
cephalexin x 7-10 days
alternatives: clindamycin, doxycycline, erythromycin, cloxacillinI
If recurrent impetigo occurring inside the nose, what could be used?
use a swab to apply mupirocin or fusidic acid TID intranasally x 3 days
What population is primarily affected by impetigo?
children
Risk factors?
neonates, young children, broken skin, close contact with patients with active lesions, hot/humid weather, comorbidities (HIV, diabetes, malnutrition)
nonbullous impetigo + MRSA suspected?
doxycycline or clindamycin or SMX/TMP
what is bullous impetigo?
fluid-filled vesicles, rupture easily, brown crusting, spontaneously can resolve in 2-3 weeks without scaring.
common areas: trunk, body folds
