Basics
Age, gender, Rx source, dx
Location of symptoms

Chief complaint
-open ended (what can I do for you? I understand your back is bothering you?)
-close down and direct interview
Description of pain
-constant: inflammatory component, go slow
-intermittent: mechanical component, may tolerate tissue movement
-throbbing: circulatory component (ex. Migraine headache, smashed thumb)
-sharp, shooting: nerve root
-intensity: scale of 0-10 to subjectively quantify
Aggravating/relieving factors
-will help in guiding pattern recognition Rx choices
-examples: intermittent pain = mechanical (move, inflammatory = rest
Behavior of symptoms
-daily/weekly cycles?
-problem at night; explore sleep habits
-arthritic; stiff in AM, relieves w/ activity
-extrinsic factors; ex of repetitive strain injuries
Special questions
-general concerns; hx of trauma (clear medically), recent or current fever/chills (clear medically)
-each region will have specific special concerns
Example of special concerns for knee, spine, and cervical spine
Knee: buckling/locking/swelling
Spine: peripheral weakness/numbness
Cervical spine: dizziness
Hx of present episode
-mechanism: acute/traumatic? Incisions/idiopathic? When and how?
-how condition evolved to this point
-long term better or worse?
-dx tests (imaging, blood/lab work)
-interventions to this point had impact?
