Immunoglobulin abnormal in SLE
IgG
Chronic multisystem inflammatory disease. Associated with abnormalities of immune system. Results from interactions among genetic, environmental and immunologic factors. Watch out for renal manifestations. Autoimmune reaction directed against the constituents of cell nucleus, DNA.
SLE
Criteria for SLE: person with presence of ________________ with ANA or antidsDNA antibodies or if 4 of dx criteria
Biopsy proven lupus nephritis
Cutaneous vascular lesions. Butterfly rash. Oral/nasopharyngeal ulcers. Alopecia.
Dermatologic manifestations
Ranging from mild proteinuria to glumerolonephritis.
Lupus Nephritis
Primary goal in treatment in SLE
slowing the progression
Is a type of antibody directed against the cell’s nuclei. Present in nearly everybody with active lupus. Used as a screening tool. Not enough for a lupus diagnosis. Very sensitive but not specific.
ANA (AntiNuclear Antibody)
Are a type of antibody directed against the phospholipids in our body. Present in 60% of people with lupus. A positive test is used to identifywomen with lupus.
Antiphospholipid Antibody
Is an antibody directed against SM. A specific protein found in the cell nucleus. Only 30% are positive. Very specific for lupus.
Anti-SM
Screening test
High sensitivity
Confirmatory test
High specificity
Protein directed double stranded DNA. Very specific for lupus. Positive test can be useful in confirming a diagnosis. For monitoring of disease progression.
Anti-dsDNA
Are two antibodies that are commonly found together. They are specific against ribonucleic acid proteins. Specific to ribonucleic acid acid. If positive, need to monitor the unborn baby.
Anti Ro(SSA) & Anti La(SSB)
Against antigens in the cytoplasm of neutrophil and monocytes.
Antineutrophil Cytoplasmic Antibodies
Wegener’s disease (60-90%)
