Front vs back
Easier to hear in back
Where to listen to
- Right under midclavical
- Middle between clavicle and nipple
- Nipple line
- Little anterior & posterior to midaxillary
- Shoot straight across to back for back spots
AAC
Causes for wheezing
Asthma
Anaphylaxis
COPD
Rhonchi
Harsh sound
Hypersecretion of mucus or infection in lungs
Most likely coughing up green or yellow
Ex. Pneumonia
Rales
“Slurping a straw”
Fluid in the lungs
What pulmonary edema sounds like
Occurs in congestive heart failure
Rales on one side could be CHF or early onset pneumonia
Bilateral + high BP + pink frothy sputum = more likely CHF
Diminished or absent lung sound
One side: pneumothorax, + hypotension means tension pneumothorax
Bilateral: Severe AAC
Stridor
Upper airway block
Will hear across the room
Think 5:
Croup - pediatric only
Epiglottitis
Inhalation burn
Anaphylaxis
Foreign body airway obstruction
Crowing
Harsh sound when inhaling
Prompt transport
