Preparing to remove victims by means of an aerial ladder can be very stressful.

A

The OV (tiller firefighter) sets chocks and stabilizers. An alternative method, when staffing is available, is to have one member assigned to the inboard stabilizer and chocks while the OV takes care of the outboard side. This would enable the OV to observe the building and be in a better position to ascend. (Hint: O-O….OV at the Outboard side)

The OV must observe the building continuously while the chauffeur is operating the ladder. Someone may appear at another window or the trapped person may move to another location. At night, use the apparatus spotlight to pinpoint and reassure the victim.

Prepare for a rapid ascent as the trapped people may attempt to climb onto the ladder without assistance. Climbing the ladder while it is being extended or retracted is prohibited.

The OV ascends the ladder followed by the CHAUFFEUR, climbs in the window, assists the victim out FEET FIRST to the chauffeur, closes the door to the fire area, and then searches the area. The victim may be unable to inform the member about other occupants.

A Lieutenant working a 6X9 tour in a Ladder designated a Ventilation Support Unit was correct in all comments made regarding utilization of positive pressure fans (PPF) for positive pressure ventilation…

A

Stairwell pressurization will not be initiated until authorized by the Incident Commander.

Pressurization of the attack stairway is most effective if done simultaneously with the fire attack.

The optimum placement of the PPF is 4 to 6 feet from the attack stairway door on the ground floor and directly facing the door to the stairwell at an angle tilt of 80 degrees.

PPFs in operation should never be left unattended; a radio-equipped member must continually monitor each operating fan. If negative results are encountered, discontinue pressurization by THROTTLING DOWN fans to IDLE, and CLOSING THE DOOR to the stairwell that the fan is pressurizing.

Two chauffeurs assigned to a busy Manhattan Engine Company were discussing pumping in high rise office buildings. All of the following are correct.

A
  • A relief valve is required at the fire pump limiting the pressure to 15 psi above that required to deliver its rated capacity to the highest floor outlet.
  • The primary water supply to the standpipe system in many buildings is a gravity tank. In many instances, there will be inadequate head pressures at the uppermost floor hose outlets supplied by that gravity tank; for this reason, the primary water supply for firefighting in most operations shall be Fire Department Engines.
  • When building fire pumps and Fire Department Engines are being used to supply the standpipe system, both systems must be monitored to prevent overheating caused by churning.
  • Building fire pumps shall be capable of delivering their rated capacity at a pressure of 50 psi at the highest floor hose outlet.

Q

Two members discussing flooring in taxpayers…

A
  • Flooring can vary in taxpayers. The most common type floors are tongue and groove boards or plywood, supported by wood floor joists.
  • Heavy terrazzo or concrete is sometimes placed over wood joist floor construction, an exceptionally hazardous condition. A fire in the cellar may weaken floor supports with little evidence of heat conditions on the first floor. These floors are difficult to ventilate to allow early advance of cellar lines or use of cellar pipes and distributors. Sudden failure due to weight on the floor is possible.
  • In newer type taxpayer slab construction, concrete floors are prevalent on ground level with no basements or cellars in the buildings.
  • Paneling under display windows at street fronts can SOMETIMES be removed to offer ventilation and stream operation points for cellar fires. The flooring under the raised display platform is SOMETIMES omitted behind these panels.

SOC Support Ladders are trained to commence operations at confined space incidents within specific limits. All limitations are correct…

A
  • SOC Support Ladder Companies shall not be permitted to attempt any entry that would require the rescuer to leave the direct line of sight of the officer supervising entry at the entrance to the space.
  • A fully equipped back-up rescuer shall remain on stand-by in the event the primary (entry) rescuer is injured, overcome, or needs assistance in escaping the space.
  • SOC Support Ladders shall only attempt an entry when all necessary precautions to safeguard the rescuer are taken, specifically, a suitable high point anchor shall be positioned to allow a retrieval system to be placed, air monitoring has taken place, and suitable steps are taken to remove hazards or otherwise protect rescuers (e.g., use of the SCBA for asphyxiation hazard).

-SOC Support Ladders shall NOT attempt any entry where the hazards are OTHER THAN atmospheric and controllable, or are not readily identifiable and controllable.
For example, a rescue of a person trapped in a cesspool is a permissible entry, since the hazards are oxygen depletion and immersion. These hazards will be controlled by the rescuer wearing an SCBA at all times during entry, by remaining attached to their retrieval line at all times, and by having a suitable hauling system in place to effect their removal in case of an emergency. Entry into a chemical tank at a tank farm, where the product and hazards are not readily known, is not a permissible entry.

There are times when Fire units may transmit on EMS borough or citywide frequencies. All are correct…

A
  • Identify yourself (e.g. “Engine 100 to EMS”)

  • Relay critical information. Understand that direct communication with the EMS borough dispatcher does not relieve units of their responsibility to maintain communications with the FD borough dispatcher as per existing Fire Department procedures.
  • Realize that a FIRE is classified as an MCI by EMS, and the assigned medical resources will typically switch to EMS-CW1 and respond on that channel.

You are first to arrive to a small flammable liquid spill that is on fire. You order your FFs to stretch and place a foam line into operation. You make several tactical decisions based on whether the spill is contained or flowing. You were correct in all…

A
  • One possible method to extinguish a spill that is contained is to order your FFs to direct the foam stream skyward in a slow back and forth motion, allowing the foam to fall as gently as possible over the surface. This will cover the spill and reduce disruption at the blanke
  • For a spill that is contained you ordered your FFs to direct the foam stream on the ground AHEAD of the fire in such a manner as to bounce the foam onto the fire.
  • For flowing fires you ordered your FFs to work against the run of the spill, either directly or from the flanks, towards the source of the spill, directing a foam stream on the ground ahead of the fire in such a manner as to bounce the foam onto the fire.
  • For a spill that is contained you ensured your FFs to deflect the stream against something and allow the foam to slide down and work its way across the burning liquid (this IS done for contained spills)

At a chemical suicide incident, responding fire units would be correct to following all of the procedures below…

A
  • Conduct a scene survey for safety, don full PPE and use the SCBA.
  • Ventilate the vehicle or occupancy prior to entering. Remove the patient from the hazardous environment and check for responsiveness. If unresponsive and the assessment meets FDNY criteria for presumed death, handle the body as part of the crime scene investigation.
  • If the assessment does not meet the FDNY criteria for presumed death, provide patient care according to CFR protocols. Remove the patient’s clothing and flush with copious amounts of water as a part of emergency decon before transport.
  • Stretch a charged hoseline with a fog nozzle attached. PRIMARILY for DECONTAMINATION; the line can also be used for vapor dispersal and fire suppression in case of ignition or explosion.

(Avoid disturbing the container, generally found in a bucket, as it may increase chemical mixing and poisonous gas emissions)
(When available, use meters to monitor oxygen levels, flammable vapor concentrations, and the presence of hydrogen sulfide, hydrogen cyanide or other chemicals)
