Recent skin infection, UTI, or IVDA. Also consider immunocompromised
Pain at rest/night
Localized
Herpes Zoster
Pain precedes rash
Osteomyelitis
Pretty much the same as discitis
Osteoporosis
Chronic pain, may be acute with compression fracture.
Work up: DEXA scan, +/- XR with
Pagets disease
Bone cells don’t function properly. Get a bone scan which shows uptake of the radioactive substance in the affected areas
Primary tumors
- Myeloma
- Sarcoma
- Osteoid osteoma
Workup for primary tumors
Start with x-ray, then may need bloodwork, CT, MRI
Types of neural tumors
- Neurofibroma
- Schwannoma
- Meningioma
Secondary tumor
aka “metastatic”
Multiple lesions seen
May start insidiously, chronic
Locations of secondary tumors
Prostate, lung, breast, kidney
Seronegative Spondyloarthropathy
Psoriatic arthritis
Anklyosing spondylitis
Inflammatory bowel disease
Reiter syndrome
psoriatic arthritis
“pencil in cup deformities”
ankylosing spondylitis
“bamboo spine” due to calcification
Reiter syndrome
calcification of the back, just more lateral on the spine
seronegative spondyloarthropathy workup
HLA-B27 and XR
