Q

Why do contact lenses need to be cleaned?

A

Build up of deposits
Lipid ( oils ) - Silicone Hydrogels = blurry vision
Protein – Hydrogels = discomfort
Hypersensivity (allergy)
Infection risk ( if too much protein build up ) - bacterial ( if you don’t clean your hands )

Q

Acanthamoeba

A

are among the most prevalent protozoa found in the environment.
Soil, air, sewage, seawater, chlorinated swimming pools, domestic tap water, bottled water = LIVES IN WATER
Highly robust
Contact lens cases - gets contaminated, if theres lots i.e. build up = lives on lens, in eye, it scratches

Q

Hydrogen peroxide

A

Metallic disc in case or tablet to neutralise the disinfectant before wear
Bleach
No adverse reaction to preservatives = no irritation
Disk at bottom, put hydrogen peroxide solution in case of disk, put lenses in, after 6 hours it will neutralise the peroxide

20
Q

What does a contact lens teach do

A
Aim to teach a new wearer
How to handle lenses
How to know if the lens is inside out
To safely apply/insert the lens
To safely remove the lens
Includes how to clean the lenses as appropriate
21
Q

Key points in a contact lens teach - insertion and removal steps

A

Touching the lens, not the eye.
Good control of top lid needed – hold lashes
Lens applied to the cornea – when teaching the public we refer to it as the coloured part of the eye
Once lens touches the eye – don’t blink straight away
Have your patient look up, down, left and right
Let go of the lashes slowly

22
Q

What to say when telling px’s to remove contact lenses

A

Slide the lens part off the cornea and pinch the lens off the sclera

23
Q

Why isn’t PMMA lenses used routinely

A

Doesn’t allow much O2 to eye

24
Q

Which lenses are used for cleaning

A

Monthlys

25
Q

What are the two types of deposits that can build up on lens

A

Protein - comes from tears

Lipid - oils attached to lens

26
Q

Which lens is good at attracting lipids

A

Silicone hydrogels

27
Q

What does protein build up cause

A

Discomfort

28
Q

What happens if you get too much protein build up

A

Hypersensitivity - almost like an allergic reaction - eyelids tend to be affected as they are in contact with lens

29
Q

What are bumps on the eyelid called

A

Papillae

30
Q

What is giant papillary conjuntivitis

A

Got hypersensitivity - feels scratch
e.g. when you have hayfever.
It should be smooth but its bumpy

31
Q

What does Acanthamoeba like to feed on

A

Cornea in the eye, however the eye is very resilient, it might get to surface of eye but never to the cornea

32
Q

Why can Acanthamoeba be very dangerous

A

If someone gets a scratch in eye and its quite deep, the protective barrier gets compromised = Acanthamoeba feed onto cornea = bad infection

33
Q

What happens if theres Acanthamoeba on the contact lens case

A

It can build up ( contaminates ), a lot of it, spread, lens in the case, it can live and eat away on the lens
