how does obesity affect breathing?
Decreases tidal volume
Reduces compliance of both the lung and chest wall
increased resp rate to allow for the decreased tidal volume
what are causes of type one respiratory failure?
PE, pulmonary hypertension, pneumothorax. chronic heart failure, ashtma
what are causes of type 2 respiratory failure?
TB, MG, polyneuropathy, head injury, hypothyroidism, OD
how do you calculate BMI?
weight (kg)/ height squared
what are the BMI categories?
<18.5 underweight
18.5- 25 normal
25-30 overweight
30+ obese
what centres in the brain control breathing?
medullary centre –> rhythm of respiration
pneumotaxic centre –> expiratory centre
Apneustic centre –> inhibits expirations
what is the chemo control of breathing?
Aortic senses partial pressures
carotid bodies sense partial pressure and hydrogen ions
how does chemo control change in COPD patients?
They are often hypercapnic so the chemoreceptors become desensitised and instead use the hypoxic drive.
what is shunt?
An area of non functioning lung means arterial blood is a mixture
what is the pathway for COPD?
- start with a SABA/SAMA
- FEV>50 add a LABA or LAMA
- . FEV <50% add a LABA and an ICS
- all three LAMA/LABA/ICS
what should be given for COPD exacerbation?
broad spectrum antibiotics oral prednisolone nebulised bronchodilators physio low flow oxygen
