top tips are a tool used to find out what is important to a person. They give 2 minutes to share what you know about the service user and the best way to support them

relationship circles are a way of identifying who is important to a person, and to explore any important issues around those relationships. It feeds into support planning because it highlights those people who should be involved in planning, and helps to discover which relationships can be strengthened or supported.

A one-page profile can help social care professionals provide better person-centred care and support. It is a simple summary of what is important to someone and how they want to be supported. On your profile you can record how you’d like people to help you, if and when you need care and support.

These are used when a person choses not to communicate with words, this allows people to understand the wants and needs of an individual.

The chart is a helpful tool for staff when making decisions in relation to a person’s care. This document also includes a blank flow chart so that staff can use the aid when making decisions within the health and social care setting they are working in. it allows the service user to have more control in their care

Developing and maintaining good relationships is central to improving outcomes for individuals and their families. Your role is to create an environment in which people feel their needs are being heard and understood. This requires integrity, honesty and skill
3.3 tools to clarify roles and responsibilities in the care
doughnut chart
It is a way to think about and identify specific roles and responsibilities of people in a situation, so that people know what must be done in a particular way and where they can experiment and be creative. It also identifies what is not part of someone’s role.
