What are the features of alcoholic steatohepatitis?
Generally unwell, low fever, tender hepatomegaly, often icteric, +/- N/V/D, ALT/AST may be greatly elevated.
Why is GGT elevated in steatohepatitis?
Hepatocellular ballooning compressing bile ducts.
Alcohol also induce biliary cells in bile ducts to produce GGT.
What is NASH?
Histologically similar to ASH but often less severe. Hepatic manifestation of metabolic syndrome.
? Greatest cause idiopathic cirrhosis
What are the common malignant liver neoplasms?
- Hepatocellular carcinoma
- Cholangiocarcinoma
- Angiosarcoma
What is the commonest liver neoplasm?
Metastases
What are the features of hepatocellular carcinoma?
-Abnormal architecture: trabeculae >3 cell thick, acini; abnormal bile canalicular architecture.
What is the aetiology of HCC?
HBC, HCV, EtOH, aflatoxin, membranous IVC obstruction, haemochromatosis, tyrosinemia.
