Liver Disease Flashcards
what is decompensated liver disease?
acute deterioration in liver function in a patient with liver cirrhosis characterised by signs and symptoms
what signs/symptoms are associated with decompensated alcoholic liver disease
jaundice, ascites, hepatic encephalopathy, variceal haemorrhage, hepatorenal syndrome
what is chronic liver disease
progressive process of hepatic fibrosis leading to hepatic architectural differences and regeneration nodule formation
what is NAFLD
Non-alcoholic fatty liver disease
what 4 risks are risks of NAFLD
TPN, HTN, obesity and diabetes
what is the pathophysiology behind NAFLD
increased fat (steatosis) –> inflammation of the liver (steatohepatitis) –> fibrosis –> cirrhosis –> liver failure
what 10 signs are related to CLD
jaundice (scleral ictarus), kayser-flesher rings, spider nevi, gynaecomastia, caput medusa, ascites, leukonychia, palmar erythema/ dupuytrens contracture, asterixis
raised IgG in suspected liver disease is usually caused by?
autoimmune hep
raised igM in liver disease is usually caused by?
primary biliary cirrhosis
raised IgA in liver disease is usually caused by?
alcoholic liver disease
what investigations do you do with liver disease
billi, ALT, ALP, GGT, Alb, PT
which viral hepatitis’ are ssRNA
HEP a,c,e
which viral hep are dsdna
b
which viral hep is circular defective ssrna
d
which hep mostly causes liver disease?
c ~80%
what does cirrhosis cause
portal hypertension and impaired liver cell function
cirrhosis pathophys
necrosis of liver cells, fibrosis, nodule formation
what is the leading cause of liver disease in the west
alcohol
what is the leading cause of liver disease world wide
viral infections
what causes more fat production in the liver
NADH converted from NAD+ from alcohol
what does ADH give off?
ros = cell damage
acetaldehyde binds to what?
macromolecules = adducts, foreign bodies
what does acetaldehyde cause/trigger ?
alcoholic hepatitis
what are the 7 causes of decompensated ALD
gi bleed, infection, alcohol, drugs, electrolyte imbalance, AKI, dehydration/constipation
when is AST more raised than ALT
Alcoholic hepatitis
what is treatment for ALD
Corticosteroids and stopping alcohol
how many stages of hepatic encephalopathy are there? what are they ?
4
1- shorted attention span, reversal of sleep-wake cycle, personality changes
2- lethargy, disorientation, asterixis, personality change
3- stupor, confusion, incomprehensible speech
4- coma
what is hepato-renal syndrome
changes to circulation + sodium and water retention and renal vasoconstriction = decrease in renal blood flow and reduced GFR
what is compensated liver disease
liver can function normally, little to no symptoms
common risks of liver diseae (3)
alcohol, obesity, hep b/c
is cirrhosis reversible
mostly no
mx of hepatorenal syndrome?
terlipressin + HAS
what scan is used to check the elasticity of the liver
fibroscan
what tests is used to assess and treat variceal hemorrhages
endoscopy
what is used to assess hepatocellular carcinoma and fatty liver
ct/mri scans
cirrhosis causes what increased risk for hepatocellular carcinoma
3-10% of patients with cirrhosis go on to developing hepatocellular carcinoma
how is hep a transmitted
faecal oral
how is hep b transmitted
bodily fluids or vertical transmission
how is hep c transmitted
bodily fluids
how is hep d transmitted
only in patients with hep b
how is hep e transmitted
faecal oral
what is type 1 and type 2 autoimmune hep found in
1= adults, 2 = children
how to treat autoimmune hep
prednisolone, azathioprine
what is wilsons disease
excessive copper accumulation in the body and tissue caused by a mutation in wilsons disease protein on chromosome 13
pathophys of wilsons disease
copper deposition = chronic hep = liver cirrhosis
sign for wilsons
kayser-flesher
tx for wilsons disease
trientene, penicillamine
test for wilsons?
serum copper, serum caeruloplasmin
acute pancreatitis mnemonic?
I: infections
G: gall stones
E: ethanol
T: trauma
S: scorpion sting
M: mumps
A: autoimmune
S: steroids
H: hypertriglyceridaemia, hypercalcaemia
E: ercp
