  • Usually originates from superficial skin lesions (ex: boils, ulcers, tine pedis) or following trauma.
  • More common in lower extremities.
  • Usually middle aged, elderly
  • Risk factors: diabetes, varicella, impaired peripheral circulation, chronic steroid use.
