LFTs Flashcards
LFTs test names:
- Bilirubin (total & direct/conjugated)
- ALP
- GGT
- ALT
- AST
- Total Protein
- Albumin
- Globulins
Functions of the liver
Fat metabolism, Glycogen & glucose metabolism, drug metabolism, bilirubin conjugation, protein synthesis - albumin & clotting factors.
Raised unconjugated bilirubin alone
Haemolysis, Gilbert Syndrome
How to test for haemolysis (3)
Anaemia, reticulocytes, haptoglobulin
Haptoglobin
binds haemoglobin and is used up even in mild haemolysis
Isolated elevated ALP (2)
Bone formation
Pregnancy - placenta secretes
Elevated ALP bony causes (4)
Young & Growing
Vit D deficiency - osteomalacia
Metastatic disease
Pagets disease
Isolated raised GGT causes (4)
Alcohol, phenytoin, metabolic syndrome, obesity
Raised ALP & GGT
Biliary Disease
Mildly elevated transaminases (ALT & AST, GGT)
Fatty liver disease, chronic hep C or hep B, haemochromatosis, autoimmune.
Fatty liver disease
steatosis - hepatic manifestation of metabolic syndrome.
high risk of cirrhosis
Marked elevated transaminases
Paracetamol Overdose
Acute viral hepatitis
Paracetamol overdose treatment
<2 hrs - activated charcoal
2-8 hrs - measure serum paracetamol & ALT - nomogram
>8 hrs - acetylcystein
Isolated AST
MI
Elevated Globulins causes
Chronic inflammation esp RA
Myeloma or lymphoma
HIV
