Investigation
Fire investigation officer FIU Statements PRO PRC Riddor HSE. Hot debrief welfare accidents near misses equipment issues
The duties of any incident commander are?
Command and control the incident Identify Hazards and manage risk. Assess resource requirements Determine an incident plan. Co ordinate and deploy resources Evaluate progress against the plan
CIAD Coor Dep-EVProg
Firefighter safety maxim
At every incident, the greater the potential benefit of fire and rescue actions the greater the RISK that is accepted by IC’s and FF’s.
Activities that present a HIGH RISK to safety are limited to those that have the potential to save Life and prevent rapid and significant escalation of the incident.
2 types of decision making strategies?
Intuitive (fast and without thinking about it)
Analytical consciously and takes time and effort
Under stands for?
Use of building Number of floors/storeys Dimensions of footprint Extent of fire damage % of each floor. Range of equipment. Tac mode
Sectorisation
Layout
1234 clockwise
5 can fit in anywhere on the clock
Building = lobby fire search sector
Bridgehead is a sector but is between lobby and fire (2floors below bottom fire floor(e.g. Fire on 10 then bridgehead on 8)
Fire sector is all floors alight + 1 above and 1 below
Each sector has a commander search, fire and lobby sector commander
Decision control process
Situation-plan action
Situation =
incident, resource, risk info
Plan=
Objectives, tactical priorities, operational tactics
Action = communication, control
Centre = why? Expectations in 15m?
Benefit vs risk?
Op discretion reasons?
Save human life
Take decisive action to prevent an incident escalating
Where taking no action will lead others to put themselves in danger
Risks?
Benefits?
WURM Why guidance N/W? Understand risks fully? Right thing to do? Minimum deviation from rules Risk v Benefit worth it?
What is an ARA?
Risk assessment?
Analytical risk assessment
Detailed and formal record of the significant findings of the risk assessment.
