Let’s others know level of participation/progress
Don’t overestimate assist
Independent
Patient does not require any physical assistance or supervision to perform activity safely and within reasonable time
Supervision
Patient requires verbal, tactile cues, directions, instructions; assistance provided to maintain safety, perform in acceptable time or technique
Min Assist
Pt performs 75% or more of the activity or requires assistance to complete the activity
Mod Assist
Pt performs 50% or more of the activity or requires assistance to complete the activity
Max Assist
Pt performs 25‐50% of the activity or requires assistance to complete the activity
Dependent
Patient requires total assistance to safely perform the activity; this may include the use of special equipment
2+ Assist
Still include mid/mod/max/dependent
