fraudulent certificate; altered license; misleading advertising; soliciting of dead bodies; removal without permission; substance abuse; unfit due to insanity; falsify death cert.; refuse to surrender body
grounds for revocation
failure to comply with state requirements; deceptive advertising; solicitation of dead bodies; failure to provide license personnel; willingly falsify records
revocation of funeral establishment license
under licensed supervision; remains conform with health codes; conform to applicable laws, rules and regulations; fulfill family wishes within reasonable limits and public policy
requirements for conducting funerals
consumer files complaint with state board; state board conducts investigation; respondent advised of charges and hearing time; respondent may reply to charges; judgement/administrative action issued; respondent may appeal
regulatory agency complaint procedure
determined by individual states; special arrangements may exist between some states; state may issue temporary permit; permanent license may be issued to a licensee after probation period, upon review by board, upon completion of examinations
