1. Physician completing it must have personally examined the patient in the last 7 days
2. Fill out box A (Serious Harm Test). Box B should NOT be filled out in the ED
3. The patient should have a mental disorder - NOT used for detention of medically ill patients

1. Past / Present Test: reasonable cause to believe that patient has caused or threatened to cause bodily harm to themselves or another person or is incapable to care for self
2. Future Test: patient suffers from a mental disorder likely to result in bodily harm of physical impairment
