Lecture 5: Clinical conditions of elbow wrist hand Flashcards
Distal Humeral fracture: Supracondylar
2deg FOOSH: fall on out-stretched hand
-posterior displacement of distal fragement
Distal humeral fracture: T or Y condylar fracture
blow to flexed elbow or longitudinal loading to extended elbow
-joint surface disruption
proximal radius/ulna fracture: olecreanon fracture
- direct trauma
- tricpes avulusion
proximal radius/ulna fracture: radial head fracture
- axial load and pronated arm
- valgus compression
ligamentous injury - elbow radial collateral
- varus stress
- uncommon
ligamentous injury - elbow unlar collateral
- valgus stress
- overuse
- trauma
ulnar collateral ligament tear
- skiiers thumb
- gatekeepers thumb
common etiology for tendinopathies
- repetitive use or eccentric strain
- micro-damage at muscle-tendon junction
- tissue response to stress/fatigue
common impairments with tendinopathies
- pain when muscle is streched, or contracted v. resistance
- pain increases with activity
- decreased strength/endurance
- tenderness to palpation
- decreased grip strength
lateral epicondylagia most commonly involves
extensory carpi radialis brevis
sometimes:
- extensor digitorum
- extensor carpi radialus longus
rarely: extensor carpi ulnaris
Medial Epicondylalgia primarily involves
-flexor carpi radialis
occassionaly:
- palmaris longus
- flexor carpi ulnaris
rarely:
- flexor digitorum superficialis
posterior tendonitis
◦ Triceps at the attachment to the olecranon
◦ Usually follows sudden severe strain to the
triceps as the arm is fully extended
◦ Uncommon
Bursitis
most commonly injured superficial bursa
- usually occurs from direct trauma
- painless
bursitis: bicipital radial bursaa
usually due to overuse
cubital tunnel syndrome
entrapment of ulnar nerve by FCU just below medial epicondyle
- acute/chronic
- increased carrying angle
- learning on elbow
- valgus stress
